THINGS YOU SHOULD KNOW ABOUT…. YOUR WORKERS’ COMPENSATION CASE (PART 4)

  1. COMPENSATION PAYMENT RECORD

 

Keep a careful WRITTEN RECORD OF ANY COMPENSATION PAYMENTS you receive and the period covered by each payment.  In the event of future delays or problems it may be useful to keep the check stub in the envelope that the check came in and write on it the date the payment was received, periods covered, and the type of payment.

If you receive form notices starting or stopping disability payments, please send us a copy unless it shows that we have received a copy directly

  1. TREATMENT, PAIN AND SUFFERING RECORD

Keep a written calendar record of the treatment, and the pain and restrictions you undergo due to your injury.  Rate your pain on a scale of 1 to 10, with 10 representing periods of time when you have excruciating/unbearable pain.  This can be used at later dates to refresh your memory.
